Nomination Form

After reading the criteria for this award, please complete the  “Support Staff Employee of the Month” rating form. All entries must be typed or clearly written, and additional pages which support your nomination may be attached. Please be concise and direct.

CRITERIA

The following statements should be characteristics of all Sahuarita Schools employees. However, the Support Staff Employee of the Month should be exemplary in all areas and must have demonstrated specific examples of each of the following:

• Strong support of Sahuarita Schools
• Outstanding improvement in work site or job related tasks
• High degree of willingness to assist other staff members
• Genuine concern for, and effective communication with, students/staff
• Contributing to positive staff morale
• Striving for personal and professional growth
• Maintaining high standards in all job related tasks
• Loyalty to the goals of our district

NOTE: The more specific your nomination is the better chance the nominee has of receiving the award. Stories and incidents which support the criteria will help us make a fair decision.
